Abstract
Talking Points: Personal Outcomes Approach – Practical Guide brings together learning from research, policy and practice to support the implementation of outcomes-focused working in health, social care and wider public service contexts. Developed by the Joint Improvement Team, the guide explains the Talking Points approach as an evidence-based organisational approach that places people using services and unpaid carers at the centre of assessment, support planning, review and service improvement. It defines personal outcomes as both what matters to the person and the impact or difference made by services and supports. The guide sets out three core elements of the approach: engaging with people through good conversations about their lives, priorities, strengths and supports; recording outcomes information in ways that are meaningful and useful; and using that information to inform individual support, service development, planning, commissioning, improvement and performance. It emphasises that outcomes-focused working is not a bolt-on activity, but requires changes in practice, systems and organisational culture. Drawing on implementation learning across Scotland, the guide offers practical advice for organisations seeking to move from service-led, tick-box approaches towards more conversational, analytical and person-centred practice. It also situates Talking Points within wider policy agendas, including personalisation, prevention, integration, self-directed support and public service reform. The guide is intended for managers, practitioners, planners, commissioners, policy leads and improvement partners seeking to embed personal outcomes at the heart of everyday practice and organisational decision-making.
