Abstract

This report documents a collaborative project between DHI, NHS Fife, and partners. The work focuses on using QR codes to hand over digital records during care home to hospital transfers, aiming to improve data sharing and coordination among care homes, the Scottish Ambulance Service, hospitals, and families.
