Abstract

This paper analyses citizen and clinical requirements for referral communication in health and care services. It reviews innovation activities, unmet needs, and organisational constraints, proposing future models for referral communications. The report synthesises findings from large-scale co-design projects, outlining common requirements for professionals and citizens, and evaluating current systems such as SCI Gateway, Docman, and OpenEHR. It presents case studies in lung cancer, dermatology, and Long Covid, highlighting the limitations of current referral processes and the potential of shared care records, service directories, and asynchronous communication platforms. The report concludes with recommendations for digital infrastructure and service transformation to enable more integrated, person-centred referral management.
