Abstract

This report presents findings from a co-design process led by the Digital Health & Care Innovation Centre (DHI), Scottish Care, and the Scottish Government, engaging care home and community professionals across Scotland. The project mapped current and future state data sharing needs, highlighting the potential for integrated digital care planning tools and citizen-held records to improve person-centred care and transitions between care at home, care homes, and NHS services.
