Abstract
This proposal outlines collaboration between Healthcare Improvement Scotland (HIS), Public Health Scotland (PHS), and DHI to enhance the eFrailty Index. The report explores integrating new data sources, such as digital telecare and person-held records, to improve early identification and population management of frail older adults in Scotland.
