Abstract
This article summarises user requirements for co-managed digital health and care, based on extensive co-design activities by the Digital Health & Care Innovation Centre. It identifies overlapping needs among citizens and professionals for personal health storytelling, data sharing, and improved dialogue. The findings highlight the need for tools that support person-centred, integrated, and preventative care, moving beyond siloed, condition-specific solutions. The article advocates for horizontal co-management across circles of care, empowering citizens to tell their story once and share data across domains. It calls for investment in digital infrastructure to support whole-of-life, person-centred models, aligning with national strategies for data, digital, health, and care.
