Abstract
This report maps digital service pathways for Long Covid care, focusing on collaboration between NHS Lothian and Chest Heart & Stroke Scotland (CHSS). It details two main routes for citizen engagement—through NHS or CHSS—and describes the systems, workflows, and data sharing required for effective case management. The report highlights the importance of shared records, self-assessment, personalised guidance, and ongoing management, as well as the challenges of integrating clinical and non-medical services. It discusses architectural roadmaps, service models, and the need for flexible, multi-entry pathways, emphasising the role of digital tools in enabling co-management and supporting citizens across organisational boundaries.
