Abstract

This report outlines the innovation and development journey of the Lothian Frailty Pathfinder Stage 4 project, part of the Technology Enabled Care Transforming Local Systems Programme. It details achievements in leveraging technology-enabled care and design thinking to create a person-centred, digitally integrated frailty care system. The report highlights partnerships, challenges, and the transition to scalable solutions across Lothian Health & Social Care Partnerships. It reviews previous phases, service design approaches, data sharing, prototyping, and simulation activities, and presents deliverables, lessons learned, and future plans. The project serves as a feeder for national programmes, aiming to inform and accelerate live deployments in integrated care.
