Abstract

This report presents a project exploring the sharing of health and care attributes with citizens to enable integrated care. The team mapped heart failure services and technologies, proposing future methods for attribute sharing across organisational boundaries. The project developed and tested prototype integrations between NHS platforms and citizen-held record systems, focusing on the use of Personal Data Stores and digital wallets. Key goals included demonstrating the movement of verified health attributes, improving trust and provenance, and embedding these capabilities within existing service experiences. The report details technical development, user journeys, and feedback from stakeholders, highlighting challenges and opportunities for national infrastructure. The findings inform future digital identity and data sharing strategies, aiming to support citizen empowerment and more holistic, joined-up care.
