Abstract

This report documents the progress of the “My Story” project, which builds on previous work from the Independent Care Review and the Digital Health & Care Innovation Centre. The project explores co-produced care records that empower young people and reduce stigma, focusing on participatory design and personal data ownership. It highlights the need for trauma-responsive design and participatory approaches to ensure care records reflect the voices and experiences of young people. The report details stakeholder engagement, design values, and insights from workshops and interviews, emphasising the importance of sense-making tools, privacy, agency, and inclusion. The project aims to prototype digital services that enable young people to tell their stories, co-manage data, and facilitate collaboration between public and third sector organisations. It outlines the design journey, engagement activities, and plans for simulation and integration with existing systems, aiming to influence policy and practice in care record management.
