Abstract
ESCP 7th spring conference on clinical pharmacy, tackling inequalities in the delivery of pharmaceutical care, 16-19 May 2007, Edinburgh, UK. The objective of this paper is to apply a qualitative research approach to develop and evaluate a standardised document-an interface pharmaceutical care plan (IPCP)-for the transfer of relevant pharmaceutical care issues (PCIs) to facilitate continuity of care after hospital discharge.
